Home care packages
A Home Care Package provides you with unlimited advice (care management) and access to all the services you need to stay in your home for as long as you wish.
From personal care, through to assistance with your shopping or garden maintenance, we’ll work with you to arrange these services on your behalf with our trusted network of service providers.
Importantly, there are no time limits on the quality advice our team of tertiary qualified, experienced Allied Health professionals provide.
There are four levels of home care packages to help meet the different levels of care needs. From Level 1 which covers basic care needs, through to high level care under a Level 4 home care package.

As a not-for-profit, all charges go back into services needed to provide quality care. Your wellbeing is our top priority, always. Everyone has the right to quality care. We provide value for money and quality care planning to allow you to remain independent for longer.

AccessCare provide government-funded Home Care Packages to people living in the Local Government Areas of Kingston, Bayside, Glen Eira, Stonnington and Port Phillip.

Planning your support
When you become a client of AccessCare, you work with an Aged Care Adviser, who helps you identify your care goals and creates a support plan that works toward achieving them.
We then help you access the services you need from high quality health and community care providers.
Throughout this process, you’re in control, you make decisions about your own care, and you can have as little or as much involvement as you wish in the management of your support plan.
AccessCare's tertiary qualified Aged Care Advisers listen to what’s important to you, supporting and empowering you to stay active and enjoy life.
Our advisers are highly valued by clients and are often used by families as a sounding board during times of change. They listen to your preferences and develop care plans that will achieve your goal as you intend.
Depending on the level of involvement you wish to have, here are some of the options available:
- Six week physiotherapy program to build strength and confidence to complete shopping independently
- Care worker can drive you to the shops and you complete the shopping together
- Care worker can shop on your behalf and bring groceries to your home
