Whether required on a short- or long-term basis, AccessCare can connect eligible clients with the care they need and tailor support that will improve their physical health and mental wellbeing.

Reablement 

We are experts in post-hospital care. Known as reablement, post-hospital care can assist clients to regain any loss in mobility or abilities that may have resulted from a fall or operation. It is short-term assistance that is more targeted towards a specific goal and aims to help clients regain confidence and the capacity to resume the activities they love. 

Dementia care

As many people with dementia are now choosing to continue living in their own homes, AccessCare is well positioned to offer support to eligible clients.

Our clients may include people who live alone with no carer, have no formal diagnosis of dementia or who are coping with other issues, such as sensory loss, family conflict or complex medical problems. 

AccessCare has developed a dementia model of care and a person-centred approach to assessment to best support our clients. Our team takes a ‘whole of person’ approach to care planning, which promotes autonomy, independence, participation and dignity.
