How to register for home care support

AccessCare client answers his front door to his support worker

Taking the first step towards receiving in-home care and support can be overwhelming. We've put together a step-by-step guide on accessing funding and assistance.

Step 1: Contact My Aged Care

Contact My Aged Care, the entry point to Australian Government-funded aged care services. You can call them on 1800 200 422 or visit their website.

The My Aged Care team will organise a free home assessment for you and arrange an Aged Care Assessor to chat with you about your day-to-day routine, current health concerns and what is important to you about your care.

Step 2: Receive the results of your assessment

Following your home assessment, you'll receive a letter letting you know if you're approved to receive Government funded care.

If you are approved for the Commonwealth Home Support Programme, you will be sent a unique referral code for each service you are eligible for. You need to give the code to your chosen home care provider (see Step 5 below).

If you are approved for Support at Home, you will be placed on the Support at Home Priority System. This system ensures that people with higher needs receive services sooner. You will receive a second letter from My Aged Care confirming your approved support and classification when funding becomes available

Step 3: Determine the cost of your package

You may still be required to make a small contribution towards the cost of your approved services. Whether or not you have any out-of-pocket expenses will depend on the type of care you're approved for and your situation.

Your AccessCare Care Partner will be able to help you understand how much you will have to contribute.

Step 4: Choose your home care provider

You have the right to choose your home care provider, which is the organisation that will deliver your home care services.

Please contact us to see what services we can offer you.

Step 5: Start receiving care

Once you have chosen a home care provider, you will need to call them and give them the unique referral code or codes you received at step 2.

With AccessCare as your home care provider, our team will tailor a care plan that suits your lifestyle and life goals. When your plan is complete, we'll start scheduling your services.

Our team is available to you 24/7 if you have any questions or if your needs or circumstances change. We are here to support you and guide you to ensure you continue to live in your own home for longer.
